Insurance lapses: what the FMCSA register shows
20,983 of 398,114 liability filings (BMC-91 and 91X) on the register carry a cancellation date, 5.3%. 1,753 active common-carrier authority records (0.58%) require liability cover and show none on file. If an insurer cancels a filing and no replacement arrives in time, the authority can be revoked. The register records filings, not whether a truck is actually insured.

The legacy FMCSA licensing files used here end in May 2026 (revocations and authority history stop then, and insurance filings taking effect fall away after that month), so a cancellation date already in the past can mean the filing is still on the register, not that the date is ahead of us. The register shows what insurers filed with FMCSA. It does not prove a policy is in force or has lapsed, and it shows no insurer or carrier here. Rises or falls in filing counts by month are not trends: only filings still active appear, so older months look thinner.
Cancellation dates on liability filings
Liability filings that carry a cancellation effective date, by the month of that date. The peak is June 2026 with 12,080 filings. Dates outside the eight months shown are grouped as earlier and later.
Counts of filings: Earlier 2; May '26 7,696; Jun 12,080; Jul 972; Aug 120; Sep 28; Oct 16; Nov 7; Dec 15; Later 43. A cancellation date on file is a notice from the insurer. The insured may already have a replacement filing, which then shows as a separate filing.
Filings by form
Each insurance form is a different kind of filing. Liability (BMC-91 and 91X) is the one tied to operating authority for most freight carriers.

|---|---|---|---|
| BMC-91X (liability, BIPD) | 392,700 | 20,875 | 5.3% |
| BMC-34 (cargo) | 35,968 | 307 | 0.9% |
| BMC-84 (surety bond) | 30,482 | 746 | 2.4% |
| BMC-91 (liability, BIPD) | 5,414 | 108 | 2.0% |

| Form 83 | 83 | 0 | 0.0% |
| Form 35 | 4 | 0 | 0.0% |
Total 467,983 filings, of which 22,119 carry a cancellation date. Forms not named here are in the file under their code and are not interpreted on this page.
Liability amounts on file
The largest amounts among liability filings on the register. This is the amount filed, not what a fleet needs for its own risk.
398,023 liability filings in the amounts counted. The most common required minimum on active common-carrier authority records is $750,000 (263,398 of 328,575 records).
Context: revocation notices, all reasons
Involuntary revocation notices entered per month in the authority history file. The file does not give a reason, so this is not a count of insurance revocations. Complete months, January 2023 to April 2026.
Totals by year in the range: 2023: 130,603; 2024: 133,207; 2025: 134,659; 2026 (to April): 43,018.

State is the business-address state on the authority record. "Both" counts records with no liability filing on file and a revocation pending; nationally that is 93% of the records with none on file. Records, not companies. A missing filing can be a timing gap in the register.
What this number means
- Nationally 5.3% of liability filings carry a cancellation date. The register alone cannot say how many are resolved by a replacement policy.
- 0.58% of active authority records show no liability filing, from 0.00% in Vermont to 0.94% in Rhode Island among states with enough records.
- A gap in filings can put an authority at risk of revocation. The register cannot show whether a truck was covered during the gap.
- The open files give no insurer names here by design, and no reason for a cancellation.
What to do about it
- Put your renewal date in the calendar 60 days ahead and ask your agent now for the new filing date. Confirm in writing that the new filing reaches FMCSA before the old one ends.
- After every renewal, look up your authority on the FMCSA licensing site and check that your liability filing is shown with the right amount. Keep a dated screenshot.
- If you pay by monthly installment, ask what happens on a missed payment and how many days of notice the insurer must give FMCSA. The notice period depends on the filing, so get it from your policy.
- If a cancellation notice arrives, call your agent the same day, get the replacement filed, and then check the register until it shows.
Renewal-week checklist
Tick these off at every renewal. They use no data from this page and need no software.
- Renewal date and filing date are both in my calendar, not only the policy end date.
- My agent confirmed in writing which FMCSA filing replaces the old one and on what date it takes effect.
- My authority status on the FMCSA site shows active after the change, with the amount I expect.
- My process agent (BOC-3) filing and my business address on the record are current.
- My broker and shipper contacts have the new certificate before the old one ends.
- I know who to call, and the number, if I get a cancellation notice.
Questions
Does a cancellation date mean my insurance has lapsed?
No. It means an insurer filed a cancellation notice with an effective date. If a replacement filing is in place the notice may no longer matter, but confirm it with your agent and on the FMCSA site. The register alone cannot show a lapse.
Can I see which insurers cancel the most?
Not here. This page uses aggregates only and names no insurer or carrier.
Why does the page not show insurance filing trends over time?
Only filings that are still active or pending are in the file, so older months have fewer surviving filings and any trend would be an artifact of how the file is built.
How much liability cover does a carrier need?
The amount depends on what you haul and on federal and state rules. In the register, the most common required minimum on active common-carrier authority records is $750,000. Check the FMCSA financial responsibility rules and your contracts for your own case.
Where do I check my own filing?
On the FMCSA licensing and insurance site, searching your docket. This page gives national and state counts and cannot show an individual record.
